Please note a change to our KS physical evaluation form regarding history of COVID-19. This is important from a medical perspective so our nurses can care for your child knowing full medical history, but this information is also important for our Athletic Trainers to know so they can help monitor for any potential post-COVID-19 complications that might arise during athletics. If your KS physical evaluation form does not have the history of COVID-19 question on the form, you have an outdated version.
